Creatine After 45—Helpful Tool or Internet Hype?

A new report, published September 9, 2026, in a supplement of the Journal of the International Society of Sports Nutrition, involved 64 healthy but sedentary adults ages 45–65.

Creatine was once discussed primarily in bodybuilding circles. Today, it is increasingly promoted to middle-aged and older adults as a supplement that may support strength, muscle mass, and even brain health.

But what does the research actually tell us?

For this week’s Science Friday, we’re sharing a newly published report that examined creatine supplementation in sedentary adults between ages 45 and 65.

Before we get into the results, we want to be completely clear: Worcester Fitness did not conduct this research. Science Friday is our opportunity to share credible health and exercise information that we believe our members, friends, and neighbors may find interesting.

What Is Creatine?

Creatine is a substance that is produced naturally by the body and also naturally absorbed through foods like meat and fish. Most of the body’s creatine is stored in skeletal muscle, where it helps support the rapid production of energy during demanding physical activity.

Creatine monohydrate has been extensively studied for its potential to improve strength, power, and training performance. Interest has more recently expanded to its possible role in healthy aging and cognitive function.

An evidence review published in the Journal of the International Society of Sports Nutrition reports that creatine is generally well tolerated by healthy adults when used at commonly studied dosages. That does not mean it is appropriate for everyone or that every claim made about it is supported by evidence. Read the scientific review.

What Did the New Research Examine?

The new report, published September 9, 2026, in a supplement of the Journal of the International Society of Sports Nutrition, involved 64 healthy but sedentary adults ages 45–65.

Participants first selected one of two lifestyle tracks:

  • A dietary intervention without structured exercise
  • The same dietary intervention combined with supervised exercise three times per week

Within those tracks, participants were then randomly assigned to receive either creatine monohydrate or a placebo for 12 weeks. The supervised program included resistance and cardiovascular training. Participants receiving creatine consumed 10 grams per day.

What Did the Researchers Find?

The researchers reported that creatine supplementation was associated with improvements in lean tissue, muscular strength, and muscular endurance among participants in both lifestyle tracks. The largest physical improvements were generally seen among participants who combined creatine with supervised exercise and dietary changes.

Researchers also reported improvements in certain measures of cognitive performance among participants receiving creatine. (Those findings were exploratory and should be treated as a reason for additional research, not proof that creatine prevents cognitive decline or improves long-term brain health.)

What Does This Mean for Adults Over 45?

The most useful takeaway may be that creatine deserves thoughtful discussion rather than hype or dismissal. The findings add to research suggesting that creatine may have applications beyond competitive sports and bodybuilding, but the study does not suggest that taking a supplement delivers the complete benefits of exercise.

Exercise provides cardiovascular, metabolic, mobility, and psychological benefits that cannot be duplicated by a supplement. At Worcester Fitness, progressive strength training, cardiovascular activity, mobility work, recovery, and good nutrition remain the foundation of a well-rounded fitness program.

A supplement may complement those habits for some people. It cannot replace them.

Why We Should Be Careful With the Results

This was a small, short-term study involving only 64 adults over 12 weeks. Other important limitations include:

  • Participants chose the exercise or non-exercise track, so differences between those groups could partly reflect motivation, health, or other characteristics.
  • The participants were healthy, sedentary adults ages 45–65; findings may not apply to active adults, people over 65, or individuals managing medical conditions.
  • The study used 10 grams of creatine daily. That is the study protocol not a dosage recommendation for Worcester Fitness members.
  • The study does not answer questions about long-term outcomes.
  • The cognitive findings were exploratory and require confirmation.
  • The report appeared in a journal supplement and does not provide the depth of a large standalone clinical-trial publication.

This is promising early research but not a final answer.

Should You Take Creatine?

That is a conversation to have with your physician, registered dietitian, or another qualified healthcare professional familiar with your health history especially if you have an existing medical condition, concerns about kidney function, or take medications.

A fitness professional can help design an exercise program around your abilities and goals. Decisions involving supplements, medical risks, and possible interactions belong with qualified healthcare providers.

Worcester Fitness Takeaway

We understand why creatine is receiving so much attention. Maintaining strength, muscle and physical independence becomes increasingly important as we get older.

This new research is interesting, but it does not change the fundamentals: begin with a program you can perform safely and consistently, include appropriately scaled resistance training, build cardiovascular fitness, allow time for recovery, and support that work with good nutrition and qualified guidance.

Creatine may prove useful for some adults. The strongest interpretation is to view it as a possible complement to healthy habits—not a substitute for them.
